New Patient Registration

There are two steps to new patient registration at TBD Clinic:

  1. Complete registration and consent form
  2. Send medical documents. This can include tick-borne disease testing, general blood test results, ECG, MRI, specialist letters, etc

Please select the relevant pathway:

✔️ Recent Tick-Borne Disease Test

Patient has tick-borne disease blood test results that are not older than 6 months.

(These results must be provided in English).

1. INITIAL CONSULTATION (1 Hour)

– Clinical history review 

– Clinical assessment

– If needed, further monitoring investigations are requested, e.g. blood tests and ECG.

2. INVESTIGATIONS

Patient completes tick-borne disease testing, and if requested, monitoring investigations and results
are provided to the clinic

3. 2nd CONSULTATION

– Discuss tick-borne diseases blood test results

– Treatment options discussed

Patient completes tick-borne disease testing, and if
requested, monitoring investigations and results are provided to the clinic

4. POST-CONSULTATION 

CONSULTATION LETTER is provided, including TREATMENT PLAN & PRESCRIPTION sent to pharmacy

To assist in ensuring safe treatment, monthly ECG and monitoring blood tests may be requested.

5. FOLLOW-UP CONSULTATION

– Clinical assessment

– Review of symptoms and recent investigation outcomes

– Provide changes to treatment plan if needed

– Prescription sent to pharmacy

Appointment scheduling 3 monthly or earlier if required.

❌ No Recent Tick-Borne Disease Test

Patient has not yet completed tick-borne disease blood testing or test results are older than 6 months. 

(These results must be provided in English).
